A patient with macular pathology, glaucoma or low corneal contrast is gently steered away from multifocal IOLs. What is cataract surgery and how is it performed in India?
Cataract surgery removes the eye's clouded natural lens and replaces it with a clear artificial intraocular lens (IOL). The standard technique across India is Phacoemulsification a 2.2-2.8 mm self-sealing incision through which an ultrasonic probe emulsifies and aspirates the cataract before a foldable IOL is implanted. The newer Femto Laser-Assisted Cataract Surgery (FLACS) uses a femtosecond laser to make the incisions, capsulotomy and lens fragmentation steps bladeless. Both are done as day-care procedures under topical (drop) anaesthesia in 10-15 minutes.
At what age should cataract surgery be done?
There is no fixed age. Surgery is recommended when the cataract begins to affect daily activities such as reading, driving at night, recognising faces or working on screens. In India, age-related cataracts typically appear after 50, but diabetes, prolonged steroid use, trauma and high UV exposure can cause cataracts earlier. The old advice to "wait until it ripens" is obsolete modern phacoemulsification is actually easier on a softer, earlier-stage cataract.
How long does the surgery take and is it painful?
The actual surgery takes 10 to 15 minutes per eye. It is painless: anaesthetic eye drops are used and no injection is needed for most patients. You stay awake, see flashes of light, and may feel a pressure sensation but no pain. You walk out the same day with a clear protective shield. The second eye is typically operated 1 to 2 weeks after the first.
How much does cataract surgery cost in India in 2026?
At our NABH-accredited partner centres in Delhi-NCR: ₹25,000-45,000 per eye for standard phaco with a monofocal IOL; ₹50,000-90,000 with a multifocal or EDoF IOL; ₹85,000-1,40,000 for FLACS combined with a premium trifocal or toric trifocal IOL. Which IOL is best for me?
There is no single best IOL it depends on your daily activities and your eye's anatomy. Active screen users and drivers often prefer EDoF (Vivity, Symfony). Patients wanting full glasses-freedom prefer Trifocal (PanOptix). Patients with astigmatism need a Toric variant. Patients with macular pathology, glaucoma, very large pupils or severe dry eye are gently steered away from multifocals and counselled towards a monofocal-plus instead. We never recommend a premium IOL if your eye isn't a fit for it.
Is FLACS (Femto Laser) better than standard phaco?
FLACS adds precision to the incisions, capsulorhexis and lens fragmentation by replacing the surgeon's manual steps with a femtosecond laser. Visual outcomes in large randomised trials (FACT, FEMCAT) are similar to standard phacoemulsification in routine cases, but FLACS is particularly useful for: dense or hard cataracts, low endothelial cell counts (corneal dystrophies), premium toric/trifocal IOL implantation where capsulorhexis centration matters, and patients prioritising the latest technology. For a soft, uncomplicated cataract in a healthy eye, MICS phaco is equally good and far cheaper.
When can I return to work, driving and travel?
Light desk work and screens: day 2-3. Driving by day: week 1 once vision stabilises and the surgeon clears you. Air travel: day 2 onwards for short flights, week 1 for long-haul. Avoid bending below waist, lifting > 5 kg, swimming, dust-prone environments and eye make-up for 3-4 weeks. Religious head-bath / hair-wash: avoid direct water on eyes for 2 weeks use a hairdresser's salon wash instead.
Can a cataract come back after surgery?
No the cataract itself is removed permanently. However, in 15-25% of patients the thin posterior capsule that holds the IOL can become cloudy months or years later (Posterior Capsule Opacification, PCO). This is corrected in 5 minutes by a painless outpatient YAG laser capsulotomy that creates a clear opening behind the IOL. It is done once and is permanent.
Will I be completely free of glasses?
Honest answer: nothing is guaranteed. Monofocal IOLs give crisp distance only reading glasses are needed. Multifocal & Trifocal IOLs aim for spectacle-independence and 80-90% of well-selected patients achieve it but a thin pair of reading glasses for fine print or low light may still be needed occasionally. We will never promise zero glasses we promise a realistic expectation backed by your specific eye measurements.
Is cashless insurance available?
Yes.
